<title><![CDATA[EULAR revised recommendations for the management of fibromyalgia]]></title>

<sec><st>Objective</st>
<p>The original European League Against Rheumatism recommendations for managing fibromyalgia assessed evidence up to 2005. The paucity of studies meant that most recommendations were &lsquo;expert opinion&rsquo;.</p>
</sec>
<sec><st>Methods</st>
<p>A multidisciplinary group from 12 countries assessed evidence with a focus on systematic reviews and meta-analyses concerned with pharmacological/non-pharmacological management for fibromyalgia. A review, in May 2015, identified eligible publications and key outcomes assessed were pain, fatigue, sleep and daily functioning. The Grading of Recommendations Assessment, Development and Evaluation system was used for making recommendations.</p>
</sec>
<sec><st>Results</st>
<p>2979 titles were identified: from these 275 full papers were selected for review and 107 reviews (and/or meta-analyses) evaluated as eligible. Based on meta-analyses, the only &lsquo;strong for&rsquo; therapy-based recommendation in the guidelines was exercise. Based on expert opinion, a graduated approach, the following four main stages are suggested underpinned by shared decision-making with patients. Initial management should involve patient education and focus on non-pharmacological therapies. In case of non-response, further therapies (all of which were evaluated as &lsquo;weak for&rsquo; based on meta-analyses) should be tailored to the specific needs of the individual and may involve psychological therapies (for mood disorders and unhelpful coping strategies), pharmacotherapy (for severe pain or sleep disturbance) and/or a multimodal rehabilitation programme (for severe disability).</p>
</sec>
<sec><st>Conclusions</st>
<p>These recommendations are underpinned by high-quality reviews and meta-analyses. The size of effect for most treatments is relatively modest. We propose research priorities clarifying who will benefit from specific interventions, their effect in combination and organisation of healthcare systems to optimise outcome.</p>

<title><![CDATA[Consensus-based recommendations for the management of juvenile dermatomyositis]]></title>

<sec><st>Background</st>
<p>In 2012, a European initiative called <I>S</I>ingle <I>H</I>ub and <I>A</I>ccess point for pediatric <I>R</I>heumatology in <I>E</I>urope (SHARE) was launched to optimise and disseminate diagnostic and management regimens in Europe for children and young adults with rheumatic diseases. Juvenile dermatomyositis (JDM) is a rare disease within the group of <I>paediatric rheumatic diseases</I> (PRDs) and can lead to significant morbidity. Evidence-based guidelines are sparse and management is mostly based on physicians' experience. Consequently, treatment regimens differ throughout Europe.</p>
</sec>
<sec><st>Objectives</st>
<p>To provide recommendations for diagnosis and treatment of JDM.</p>
</sec>
<sec><st>Methods</st>
<p>Recommendations were developed by an evidence-informed consensus process using the European League Against Rheumatism standard operating procedures. A committee was constituted, consisting of 19 experienced paediatric rheumatologists and 2 experts in paediatric exercise physiology and physical therapy, mainly from Europe. Recommendations derived from a validated systematic literature review were evaluated by an online survey and subsequently discussed at two consensus meetings using nominal group technique. Recommendations were accepted if &gt;80% agreement was reached.</p>
</sec>
<sec><st>Results</st>
<p>In total, 7 overarching principles, 33 recommendations on diagnosis and 19 recommendations on therapy were accepted with &gt;80% agreement among experts. Topics covered include assessment of skin, muscle and major organ involvement and suggested treatment pathways.</p>
</sec>
<sec><st>Conclusions</st>
<p>The SHARE initiative aims to identify best practices for treatment of patients suffering from PRD. Within this remit, recommendations for the diagnosis and treatment of JDM have been formulated by an evidence-informed consensus process to produce a standard of care for patients with JDM throughout Europe.</p>
